TIA patients are at substantial short-term risk for developing a stroke. Both clinical and imaging risk factors can stratify this risk, identifying TIA patients at higher or lower risk for subsequent stroke. Clinicians should be aware of these predictors to devise and implement appropriate stroke prevention regimens.
Highlights:
1. TIA and minor stroke are clinically the same because they are difficult to tell apart, are caused by the same mechanisms, have the same prognosis and the same treatment.
2. TIA and minor stroke carry a 5-20% risk of major stroke within 90 days with most of the risk accruing in the first few days.
3. The risk of major stroke can be risk stratified using clinical factors.
4. Brain and neurovascular imaging can refine risk stratification.
5. Optimal acute prevention is actively being studied in randomized controlled trials.
